Advance Care Planning is the process of planning for future healthcare options and expressing your values and preferences should you become seriously ill and unable to make decisions for yourself. Learn more about this often neglected but important aspect of care for oneself and one's loved ones at this informative talk.

In this session, you’ll learn:

  • What is Advance Care Planning (ACP)
  • Importance of Planning
  • Myths and Truths about ACP
  • Differences between Will, ACP and Lasting Power of Attorney (LPA)
  • How to conveniently sign up for ACP with Tele-ACP option
