1. Find out about Lasting Power of Attorney (LPA) and Advanced Care Planning (ACP) – why these are important and complementary plans that will give you and your loved ones a peace of mind.

2. Use the My Legacy portal (MyLegacy.gov.sg) to complete your LPA and ACP forms quickly and easily.

3. Enjoy a convenient referral to SATA CommHealth clinics to formalize both the LPA and ACP.
