Come face-to-face with members of the palliative care team who care for patients with serious illnesses and their families. Hear them share moving stories of their journey of care, and experience the power of music in healing with a simple interactive activity with our music therapist.

Witness palliative care first-hand as you go on a guided tour of Assisi Hospice. Explore unique features of the inpatient ward; see the Day Care service in action as patients engage in therapeutic programmes.

Meet the Journey Guides

Dr Daniel Lim, Resident Physician, Assisi HospiceDr Daniel graduated from the University of Tasmania (UTAS) and began his medical career in Singapore in 2018. His interest in Palliative Care led him to join Assisi Hospice in 2022, where he currently serves as a Home Care doctor. In this role, he provides compassionate medical care to patients with serious illnesses in the comfort of their own homes. He finds deep meaning in the privilege of journeying alongside patients and their families during the final stages of life. His patient encounters continue to inspire him to grow, reflect, and give his best to those he serves.Mr Phang Xiao Feng, Senior Medical Social WorkerXiao Feng is a Senior Medical Social Worker with the Palliative Home Care team at Assisi Hospice. He is a registered social worker (SASW) with over 12 years of diverse professional experience. His background includes providing psycho-emotional support in acute hospital settings like the Intensive Care Unit (ICU) and emergency department, as well as in an early intervention centre for children with special needs. This broad foundation has equipped him with a comprehensive understanding of families' needs across the healthcare and social service landscape.His current focus is on supporting palliative patients and their families within the home care setting. Xiao Feng uses the biopsychosocial-spiritual (BPSS) framework to holistically understand each person's journey with life-limiting illnesses, applying a person-in-environment perspective to his interventions. His work is further guided by contemporary grief theories such as the dual-process model and meaning-reconstruction.Committed to a person-centered approach, Xiao Feng pays close attention to the unique needs of the individuals and families he supports, helping them to navigate loss and find new ways of coping.Ms Elena Binte Mohamed Rahim, Senior Staff Nurse, Home CareElena graduated from Nanyang Polytechnic with a Diploma in Nursing. She worked in an acute hospital for 8 years, before venturing into palliative inpatient care in a community hospital ward. She decided to pursue her interest further, obtaining a Specialist Diploma in Palliative Care. She joined Assisi Hospice Home Care team in 2020. Ensuring that patients pass away in comfort and helping their families find the closure that they need motivate her and keep her going.Ms Sheena Low, Day Care ManagerSheena graduated from Singapore Institute of Technology with a Bachelor of Science with Honours in Occupational Therapy. She has been working as an occupational therapist since 2013 in a restructured hospital before joining Assisi Hospice in 2017. Through early integration of palliative care and adopting an interdisciplinary approach, she believes that the care team can help patients with serious illness live meaningfully by understanding them as a person. Currently serving as the Assisi Hospice Day Care Manager, she has taken the lead to realise her vision of how palliative day care model should look like in Singapore. Her patients keep her going - every story is unique and there’s always something to learn from every patient she meets.Ms Trudy Chua, Senior Music TherapistTrudy is a Registered Music Therapist (RMT) with a Master of Music Therapy degree from the University of Melbourne, and a Postgraduate Diploma in Counselling Psychology. Her journey at Assisi Hospice began in 2018 as a student music therapist, and 2019 as a professional RMT. Her clinical experience in hospice and palliative care includes working with patients in the inpatient, day care and home care settings, and clients receiving bereavement care. She adopts a humanistic approach in her clinical work, recognising the power and potential in every person, and the wonders that can unfold in a partnership with music. Trudy is immensely grateful for the chance to share in the journeys of her patients and their loved ones, bearing witness to the strength of the human spirit and the depth of love that shines forth.
