Living, Loving, Leaving @ Home

This webinar explores Compassionate Discharge - a patient-centred approach that supports families in honouring their loved one’s wish to spend their final moments at home. Participants will learn how healthcare teams and home hospice work hand in hand with families, offering guidance and support to ensure dignity, comfort, and peace in their loved one’s final hours.


Going Home

This segment of the webinar will guide participants throughthe documentations and processes needed when the time comes for loved ones at home. At the same time, the speakers will introduce various things to look out for so that families experience a smoother, well-informed and lessstressful journey.

✨ About the speakers

Mr Justin Tan

Justin joined St. Andrew’s Community Hospital (SACH) in 2017 as Associate Chaplain, bringing with him 5 years of pastoral care experience. In the specialised field of pastoral care, Justin is also a certified Thanatologist and serves as a member of the SACH Palliative Care Team, which works with patients and their families to develop personalised care plans for those with life-limiting illnesses. Journeying alongside them, the team also provides support in grief management and aspires to deliver the best possible quality of life for patients, regardless of the limitations of their illness.An advocate of patient-centric care, a core aspect of Justin’s work is to aid patients and their families in planning for end-of-life care in a manner that dignifies both living and departing. Providing pastoral care and emotional support, he believes strongly in encouraging patients to celebrate life at every opportunity. This may take the form of birthday and festive celebrations, and even seeking out what they desire so as to fulfil their last wishes.


Mr Darren Cheng

Darren has been the CEO of Direct Funeral Services for a decade. Under his supervision, he and his team has impacted more than 15,000 families. Today Direct Funeral Services is the leader in bereavement care. He also holds a masters in counselling and has created various tools to help clients cope with grief. He has written 3 books on the topic of grief for children and is passionate about helping children navigate loss
