💡 What can you expect from this session?

The Magic Carpet Trail is an interactive workshop for participants explore the concept of BPSS (Biopsychosocial-Spiritual) or Total Pain through the universal language of wishes. Each “wish stop” on our journey invites participants to reflect on a different aspect of total pain.

Through this creative and compassionate approach, we aim to showcase how the multidisciplinary team comes together to fulfil the diverse wishes and needs of those under palliative care.

✨ About the Facilitators

Dr Tan Hong Weng, MBBS, GDFM, M MED(FM), GDPM

Consultant

Dr Tan Hong Weng is a dedicated Family Physician with nearly 20 years of experience practising medicine. He is a passionate advocate for patient autonomy and person-centred care, and is committed to providing holistic care to his patients and their families.

Over the years, Dr Tan has developed a strong interest in palliative care, firmly believing that everyone deserves the opportunity to “Live Well” and “Leave Well.” Apart from taking care of our patients in our Community Hospital, he leads the palliative care workgroup at Ren Ci, where he played a pivotal role in establishing the Inpatient Hospice and Palliative Care Service.

Dr Tan, together with his dedicated team, supports patients and their families in navigating complex, life-limiting illnesses. He places the utmost importance on preserving dignity, ensuring comfort and enhancing quality of life, guiding each patient with compassion, clarity and respect.

Ms Nadirah Aseelah Chee, Senior Medical Social Worker

Nadirah is a registered Senior Medical Social Worker at Ren Ci Hospital's Psychosocial Service Department, bringing a wealth of experience from Family Service Centres, Early Intervention Programs for Infants and Children and the ILTC sector. She focuses on palliative care and grief work, incorporating creative arts into her practice to support emotional healing and personal growth.

Guided by family systems principles, meaning-making theories, and strengths perspectives, Nadirah empowers individuals and families to navigate the complexities of life-limiting illnesses. Her innovative group programs in palliative care provide essential support to clients, caregivers, and healthcare professionals, addressing both personal and professional challenges with compassion.

Ms Yee Xianfang Jolene, RMT, Music Therapist

Jolene is a Music Therapist with the Psychosocial Service Department at Ren Ci Hospital and holds credentials as a Registered Music Therapist (RMT) and Neurological Music Therapist (NMT). She uses an eclectic approach to support elders in long-term care, palliative and community hospital settings to address goals including relationship completion, grief and loss and self-expression. Jolene has also introduced music therapy services for residents living with terminal illnesses and disorders of consciousness and their families. She adopts a resource-oriented approach, recognising the potential in every person and the importance of journeying alongside others, supporting and holding space with music.

Lingaraj Prabha, BSc Nursing, Adv Dip Orth Nursing, Spec Dip (Palliative Care)

Senior Nurse Manager

Prabha is a dedicated palliative care nurse with a heartfelt commitment to easing pain and bringing comfort to those facing serious illness. Since 2016, she has walked alongside patients and families, offering care that’s both clinically and deeply compassionate. Prabha believes in shared leadership and teamwork, working closely with others to create care plans that reflect each patient’s values and wishes. Her gentle presence, thoughtful listening, and focus on symptom relief help patients find peace, dignity, and strength in every stage of their journey.

Nur Adilah Binte Mohd khalem, BSc (Hons) Occupational Therapy, SIT

Occupational Therapist

Adilah finds joy and purpose in her work as an Occupational Therapist to enable valued routines, facilitate legacy projects or wishes, or adapt activities and recommend equipment for comfort and ease, so that her patients can continue to continue to engage meaningfully in relationships and activities that matter most to them. What matters most differs for each person, and discovering these priorities with care, in collaboration with patients and their families, is at the heart of a person-centred approach in palliative care.
